Download this form and fill it out, then ensure it has your adviser’s signature . Email the signed form to gradschool_research@uic.edu.ph to secure the Dean’s signature. Once the Dean has signed the form, submit it to the Research Ethics Committee (REC) at rec@uic.edu.ph
